LEARNING you or someone you love has been diagnosed with a life-limiting illness is unimaginable and knowing what to do next can be overwhelming.

Palliative Care Connect is a new South Australia-wide service that provides information and connections to palliative care and bereavement services for individuals with life-limiting illnesses, as well as their families, friends and carers.

Funded by the Australian Government Department of Health and Aged Care, Palliative Care Connect offers a free phone service and a website designed to guide and support individuals in accessing the care they need.

Through a phone service, Palliative Care Navigators provide information, connections to appropriate services and empower individuals to make choices that align with their cultural and personal preferences.

Developed in partnership with Flinders University, the Palliative Care Connect website (palliativecareconnect.com.au) serves as a comprehensive resource hub.

It offers key information about palliative care and bereavement, a service directory, and a self-assessment tool to help identify the support and resources you may need.

Sonder, a not-for-profit organisation, has been contracted to deliver Palliative Care Connect’s Regional-Specific Navigation Service throughout areas in regional South Australia, including Barossa Light and Lower North, Yorke, and Mid North.

To speak with a local Palliative Care Navigator or request a callback, phone 1800 596 799 between 8.30am and 4pm, Monday to Friday.

In addition, you can access in-person support at Sonder’s offices in Kadina, Clare, Nuriootpa and Port Pirie.
